I thought he might be attempting some sort of trick at first. Because I doubt he truly holds any caring for me.
But it is probably because under all of the self-loathing and despair towards everything is someone who wants to be the hero. But his destructive actions mean he never will be and thus the cycle of hate and poor choices.

Trevor doesn't actually want to be helped, your efforts would be in vain no matter what. Don't put so much of yourself out there to someone like that. He will not repay your efforts.
Your emotions pull you to try and help everyone, but in some cases, you cannot. And it will only frustrate you the more you try.
Trevor will have decide whether or not he actually wants to be a hero or someone just pretending in a futile attempt to make himself feel better about who he is.
You can't make that decision for him.
He may have to lose it all before he can see this for himself.
